Requests for Security Deposit for Housing Choice Voucher
Shelter and Transitional Housing providers may complete this form to request assistance with a security deposit for clients currently in their shelter or TH program. In order to qualify for a security deposit, the client must have completed the briefing with MSHDA and identified a unit that has passed the HQS inspection (please do not submit this form until these activities are completed). Please note: Availability of funds for security deposits is driven by Rapid Re-Housing providers, and there is no guarantee funds will be available for every client.

VI-SPDAT Forms
Family VI-SPDAT
SPDAT assessment containing questions to be asked of family consumers. Score will determine consumer’s vulnerability index.
Single VI-SPDAT
SPDAT assessment containing questions to be asked of single consumers. Score will determine consumer’s vulnerability index.
Detroit Release of Information
Form outlining why the CAM needs certain consumer information, as well as how the sharing plan allows multiple agencies to view said information to help the consumer. Consumer rights must be read and initialed. Additional sharing plans are included on pages 4 and 5.
Verification Forms
Emergency Shelter Letter
Form to be completed by shelter staff verifying that consumer is residing in an emergency shelter.
Transitional Housing Letter
Form to be completed by agency staff verifying that consumer is in a transitional housing program.
